However, there’s a catch—one so significant that it risks becoming the next silent crisis in business: we are installing intelligence into systems that we do not fully comprehend.

Most organizations begin with surface-level automation. Teams experiment with tools that write code, summarize meetings, and generate content. These tasks are real, and reducing their burden has value. But to confuse this with transformation is to miss the deeper shift. The real promise of AI is not the automation of tasks, but the redefinition of work itself—how decisions are made, how people coordinate, how success emerges across teams and time.
Yet when organizations move beyond basic use cases into AI-driven coordination or decision-making, failure rates spike. A recent global study found that 85% of enterprise AI initiatives fail to deliver expected outcomes. That failure isn’t due to flawed algorithms or a lack of computing power. It’s rooted in a more fundamental problem: most companies don’t actually know how they work.
The System Beneath the System
What’s visible—org charts, strategy documents, KPIs—gives the illusion of clarity. But the real drivers of performance lie elsewhere: in the invisible web of informal commitments, unspoken dependencies, interpersonal trust, and tacit agreements that determine whether progress happens or stalls. They take time to understand how value is actually created—how trust is built, how decisions are made, how alignment is formed through human interaction. They uncover the hidden architecture of work, not just the visible one.
Only then do they begin to bring in AI—not to replace the human system, but to reflect it, support it, and extend it with integrity.

The Pattern Repeats
Just like with blood transfusions, cholesterol, and lead (see last week’s Substack)—the problems that go unseen aren’t harmless. They’re just misattributed. The unpredictable results we explain away—the missed deadlines, the stalled initiatives, the teams that suddenly fall apart—often aren’t random. They’re symptoms of a deeper rhythm we haven’t yet learned to hear.
AI has the potential to surface that rhythm. To illuminate the hidden mechanics of performance. But only if we are willing to question what we think we know and begin to see what’s always been there.
Because the real risk today isn’t that organizations are missing out on AI. The real risk is that they are moving faster than they understand—accelerating into complexity with a false sense of clarity.
The Cost We Don’t Talk About
And when that happens, the damage doesn’t just show up in decisions. It shows up in people.
There’s one more consequence, one more cost, that’s often overlooked. It doesn’t show up in quarterly reports. It isn’t flagged in operational metrics. And it’s rarely named directly. But it may be the most profound unintended outcome of all: the erosion of trust.
When AI is deployed without understanding the human structure it’s entering, something foundational begins to shift. People start to question decisions they don’t understand. Roles that once felt clear begin to blur. Accountability grows fuzzy, passed between humans and systems with no clear line of ownership. Promises are made by tools that don’t feel the weight of follow-through. And over time, the relational tissue that holds the organization together begins to tear.
Trust—between colleagues, between leaders and teams, between the organization and its customers—doesn’t disappear overnight. It erodes silently. Slowly. And by the time it becomes visible, the damage is already deep. The people who held things together quietly walk away. The customers who once waited patiently begin to look elsewhere. And the AI won’t tell you that. Because the system it’s working within doesn’t know how to measure what it never learned to see.
